Athletic Clearance Steps

All athletes trying out for any team or sport must be medically cleared and have medical insurance

PHYSICALS MUST BE DATED AFTER JUNE 13th, 2026

  1. Visit- homecampus.com
  2. Click on the tab, For Parents & Students at the top of the page
  3. Select California
  4. Start registering your Athlete
  5. Login
  6. If you are new, hit create an Account and Register- Parents/Guardians register with first and last name, valid email username and password. If you are returning login using your email and password
  7. Select “Start Clearance Here” (top right)
  8. School- Type in Mira Mesa and click on drop down box
  9. Choose the school year (2026-2027)
  10. Choose Sport- Add all the sports your student plans to play by clicking on Add New Sport
  11. Complete all required fields for Student Information, Medical History, Parent Guardian Information and Signature Forms. All athletes must have Medical Insurance to participate in athletics.
  12. Print “PHYSICAL FORM” at the bottom of the Medical Questions Page and take to your physician for a physical. Take the form with you to your doctor.
  13. In order to complete the online registration, you must scan and upload your completed physical (all 5 pages). Once uploaded hit the Submit Completed Application button at the bottom of the page. When you do that the “Confirmation” Page will pop up. Print and sign the Confirmation Page.
  14. Go back to the Files and upload the signed Confirmation page then email Ron Lardizabal that you have completed the online registration. We do not get notified by Home Campus that you have completed the registration so we cannot clear your student until we get notified by you.
  15. As Always, if you are having issues scanning and uploading the documents, bring all the documents to the office to get cleared.
